The causes
Why applications actually bounce
No federal tax return attached, when no exception applies. The W-7 instructions are explicit that applications include a return unless you meet an exception.
Expired documents. The IRS requires identification documents to be current at the time you submit.
Wrongly certified copies. A certified copy comes from the original issuing agency with its official seal. A notary stamp does not qualify, outside one narrow military exception.
